Chronic rejection after lung transplantation (LTx) leads to the bronchiolitis obliterans syndrome (BOS), wich is,characterized histologically by airway epithelial cell apoptosis and luminal fibrosis in the respiratory bronchioles causing airflow obstruction. BOS is irreversible and is the main cause of morbidity and mortality after LTx. This syndrome occurs also in 5-10% of patients with chronic graft-versus-host disease after stem-cell transplantation (SCT). The main purpose of this study is to identify patients at risk for developing BOS by defining the specificity and function of autoantibodies associated with the onset and pathogenesis of BOS. By this, we aim to identify patients at risk for developing BOS which can be used to guide immunosuppressive therapy after LTx or SCT.
In kidney transplantation, pretransplant risk assessment of graft failure is important for donor selection and choice of immunosuppressive treatment. We are currently engaged in large-scale studies on the clinical relevance of HLA-antibodies, detected by luminex prior to transplantion. In addition, the ability of the transplanted organ to protect itself from complement mediated damage is investigated.
