Astrid Koffrie

Astrid Koffrie is the secretary of the Patient Safety Center.

Pam Kato

Pamela M. Kato is a health psychologist. She has a masters in counseling and consulting psychology and a Ph.D. in psychology. Before joining the UMC Utrecht as a senior research scientist, she was the founding president and CEO of HopeLab and clinical instructor in Pediatrics at Stanford Hospital in the United States. At the UMC Utrecht Patient Safety Center, Kato is currently working on developing and testing a clinically validated serious game to train medical residents in patient safety. Cor Kalkman

Prof Dr C.J. Kalkman, MD, PhD is professor of Anesthesiology research and head of Center for Patient Safety of the UMC Utrecht.

Ian Leistikow

Ian Leistikow MD has been coordinator of the patient safety program within the University Medical Center Utrecht (UMC Utrecht), the Netherlands since 2003. This is a 1000 bed tertiary care center with 10.000 employees and an annual budget of over 750 million Euros. Leistikow has written many articles on patient safety related topics and a book on Root Cause Analysis. He has set up several patient safety training programs and is currently finishing a PhD thesis on how to manage patient safety.

Carl Moons

Karel G.M. Moons is Professor of Clinical Epidemiology at the Julius Center for Health Sciences and Primary Care, of the UMC Utrecht. He is heading the UMC Utrecht Research Programme ‘Theoretical Epidemiology and Biostatistics’. He was appointed full Professor of Clinical Epidemiology in 2005. Since 2005 he also became appointed Adjunct Professor at VanderBilt University, Nashville, USA and visiting Professor at Tokai University, Japan and McGill University Canada. He is a member of various research committees, including the National patient safety committee of the Netherlands Organisation for Scientific Research, as well as the local Medical Ethical Committee. Karel Moons is scientific reviewer of various medical journals, and associate editor of Clinical Chemistry. As a methodologist, he is also affiliated at the UMC Utrecht Center for Patient Safety.

Loes

Dr. L. Pijnenborg, MD, PhD, is Project Manager of the HANDOVER project, and staff member of the Executive Board of Directors of the UMC Utrecht. She was Programme Director of Better Faster, a nationwide program to improve Patient Safety and Logistics in hospitals in the Netherlands. lees meer
