EXPLICIT-stroke: Explaining Plasticity after Stroke
Summary research:
EXPLICIT-stroke is a multidisciplinary study in collaboration with the UMC Utrecht, VU University Medical Centre, LUMC in Leiden and UMC St. Radbout in Nijmegen in cooperation with the TU Delft. It is a study on early intensive rehabilitation training to promote recovery of the upper extremity in patients suffering from an ischemic stroke resulting in paresis of the upper extremity. Dependent on the prognosis regarding recovery, therapy consists of CIMT (constraint induced movement therapy) directed at forced use of the paretic arm, and EMG/NMS (electro stimulation of the muscles of the lower paretic arm) to promote hand extension.
Patients will be followed over 6 months, while being assessed with clinimetrics, kinematics, neuromechanics, TMS and fMRI. Patients are measured with MRI at three occasions (1 week, 1 month, and finally 6 months post stroke). The protocol consists of: fMRI (motor task, resting state), DTI (diffusion tensor imaging), in which integrity of white matter tracts are assessed and ASL (arterial spin labelling), in which changes in perfusion of the brain are measured. A total of 180 patients will be included in the EXPLICIT program, of which 30 are assessed with serial MRI.. fMRI scans session will be executed in Utrecht as well as Leiden by Floor Buma.
