Projects

1. A Nursing Rehabilitation Guideline for stroke patients
Stroke is the third most common cause of death in the western world. In the Netherlands the annual stroke incidence has been reported to be 1.74/1000/year (4203). This incidence of stroke is even more severe when one closely studies the personal disruption created by such a catastrophic event. Therefore, it is important to seek ways of effectively managing stroke while assuring quality patient care. Because stroke patients experience a wide range of problems, the rehabilitation needs of these patients are diverse and complex, requiring specific rehabilitative interventions. Nurses may play a more active role in the rehabilitation and early detection of problems in these patients. Current guidelines are of little help for nurses. Also, the rehabilitation method most commonly used currently is based on old theories and has been found not to be effective (Hafsteinsdóttir 2005). Therefore, there is a need for a nursing rehabilitation guideline for patients with stroke. The aim of this project is to develop an evidence based rehabilitation guideline, that offers a systematic overview of the research literature and findings and the evidence provided for rehabilitation nursing interventions for patients with stroke. The methods of SIGN, Nursing Science department UMC, Utrecht, AVVV, AGREE and Cochrane are used.

The guideline is developed by:

Drs. M. Rensink, researcher, Faculty chair for people with chronic illness, University of Professional Sciences, Utrecht
C. Schoneveld, clinical nurse specialist, department of Neurosurgery, UMC Utrecht
Y. Ubels, clinical nurse specialist, department of Neurology, UMC Utrecht
Drs. I. Uitewaal, clinical nurse specialist, department of Psychiatry, UMC Utrecht
F. Gooskens, clinical nurse specialist, department of Psychiatry, UMC Utrecht and Nursing Science-student
J. de Man, nurse practitioner, department of Neurology and Nursing Science-student
This is a collaboration project between the Division of Neuroscience, UMC Utrecht and the Faculty Chair for People with Chronic Illness, University of Professional Sciences, Utrecht, The Netherlands and the Landspítali University Hospital in Iceland.

2. A nutritional nursing guideline for stroke patients
Due to the high incidence and prevalence of malnutrition in patients with stroke (16%-62%), we set out to develop an evidence based nutritional guideline for patients with stroke, that nurses can use in the daily care of these patients. The aim of this project is to develop an evidence based rehabilitation guideline, that offers a systematic overview of the research literature and the evidence provided for nutritional interventions for patients with stroke. In this project we use the same method as presented for the rehabilitation guideline. The methods of SIGN, Nursing Science department UMC, Utrecht, AVVV, AGREE and Cochrane are used.

This guideline is currently being developed by:

S. Sigurjónsdóttir BSc, department of Neurology, Landspítali University Hospital in Iceland
M. Klinke BSc, department of Neurology, Landspítali University Hospital in Iceland
K. Björvinsdóttir BSc, department of Neurology, Landspítali University Hospital in Iceland
D. Bergs MSN, rehabilitation centre, Grensás, Landspítali University Hospital in Iceland
This is a collaboration project between the Division of Neuroscience, UMC Utrecht and the Faculty Chair for People with Chronic Illness, University of Professional Sciences, Utrecht, The Netherlands and the Landspítali University Hospital in Iceland.

3.Nutritional status of patients with neurological conditions
The aim of this study is to gain some insights into the nutritional status of patients with neurological conditions admitted to the Neurological and Neurosurgical wards of the UMC Utrecht.

This study will be conducted by:

C. Schoneveld, clinical nurse specialist, department of Neurosurgery, UMC Utrecht
Y. Ubels, clinical nurse specialist, department of Neurology, UMC Utrecht

4. Depression in stroke
Post stroke depression (PSD) is common in patients after stroke (16%-61%) and has negative impact on patients’ participation in the rehabilitation and the associated rehabilitation outcome. Therefore, early detection of PSD is emphasized in the literature. Antidepressant treatment improves functional status and health related Quality of Life and lowers mortality up to 9 years.
These are two literature studies focusing on instruments to detect depression in stroke patients and on therapeutic interventions for depression in stroke patients.

The literature studies are being conducted by:

Floor Gooskens, clinical nurse specialist, department of Psychiatry, UMC Utrecht and Nursing Science-student
Janneke de Man, nurse practitioner, department of Neurology and Nursing Science-student
