Voor verzendingen binnen Nederland verwijzen wij u naar de Virologie website van de Afdeling Medische Microbiologie. Hier vindt u afname- en transportinstructies. Verzenden van materiaal ten behoeve van oogvochtdiagnostiek kan met speciale verzendpakketjes, die opgevraagd kunnen worden op telefoonnummer 088-75 584 35 of via email (uveitislab@umcutrecht.nl).
For shipment from within the Netherlands, please see the instructions on the Virology website of the Department of Medical Microbiology. Ocular fluids and serum samples can be shipped using the dedicated transport packages for diagnostic materials. These can be requested by phone (088-75 584 35) or by email (uveitislab@umcutrecht.nl).
From Belgium
Voor verzendingen vanuit Belgie kunt u gebruik maken van het minimailbox systeem. Deze boxen voldoen, mits correct gebruikt, aan de P650 verpakkingsinstructies en zijn alleen geschikt voor diagnostische materialen Categorie B (UN3373). De instructies kunt u hier downloaden, maar worden ook meegestuurd.
For shipment from Belgium you may use the minimailbox system. These transport boxes are in compliance with P650 packing instructions, provided they are used correctly, and are suitable for Diagnostic Specimens Category B (UN3373) only. Instructions can be downloaded here, and are included with the minimailboxes.
From other countries
Sample identification and order form
Please mark each material-containing tube with:
name of the patient
date of birth
date of sampling
type of material
The order form should contain the following information:
patient's name
address
date of birth
name of treating phycisian or contact, including phone number and/or email
billing address
type of material
date of sampling
optional: clinical data, medication
Transport
Ocular fluids are preferably send on dry ice, although for most applications overnight express mail at room temperature is acceptable too. For overnight express mail, dedicated transport boxes (in compliance with P650 packing instructions) are available. These boxes are for Biological Substances Category B only and can be requested by email (uveitislab@umcutrecht.nl).
If using the minimailbox please follow these instructions:
1. The diagnostic material is placed in the primary receptacle (the watertight tube), which is placed in the blister containing absorbent material. Do not forget to properly mark the tubes.
2. The blister is placed in the high pressure resistant safety bag, and the bag is sealed firmly after removing the white protection stripe from the autoadhesive strip.
3. The bag is placed, together with the filled-in order form (see below), into the minimailbox.
4. Write the full address and phonenumber of the sender on the back of the minimailbox.
