Beschrijving Onderzoek: The main focus of the Department of Respiratory Medicine is “Chronic Inflammatory Pulmonary Diseases”.
Mission statement: To improve our knowledge of the pathogenesis of inflammatory (lung)diseases such as allergic asthma, COPD, interstitial lung diseases, chronic rejection after lung transplantation and cystic fibrosis with resultant improvements in the monitoring and the treatment of these diseases. Furthermore, to contribute in drug development and targeting in chronic inflammatory diseases.
Main lines of own research: Asthma and COPD.
This line of research is focused on the chronic innate immune response that plays an important role in chronic inflammatory respiratory diseases. The line is organized around both a fundamental approach and a translational approach. The fundamental line is focused on those activation signals for inflammatory cells that are also found in vivo. This focus is chosen because the chronic innate immune response is an important determinant in the pathogenesis of COPD and is also important in asthma, especially in difficult-to-treat asthmatics that constitute around 15% of all asthma patients.
The research lines are organized to gain more insight into the underlying mechanisms of the diseases involved and to develop strategies for novel therapy and diagnostics.
In collaboration with other departments outside the UMC Utrecht, but mainly within the ABC cluster, the department participates in (Pharmaco)epidemiological studies aimed at elucidating use and possible side effects ofdrugs for asthma and COPD.
At a national level, the TiPharma initiative has funded two large COPD studies, collaborations between our department and the UMCG and the AZM together with different industrial partners. The aim of this collaboration is to elucidate fundamental aspects of the local and systemic inflammation in COPD.
The pulmonary function laboratory has started a research program into early diagnosis of mild COPD to stabilize the disease in that mild stage and prevent irreversible deterioration. The studies performed are aimed to establish 1]the best combination of pulmonary function tests to separate asthma from COPD; 2] the use of carbon monoxide and nitric oxide diffusion capacity to diagnose mild emphysema, 3] the use of exhaled volatile organic compounds to diagnose mild emphysema and 4] the value of classical pulmonary function testing to diagnose mild emphysema.
Within the large Nelson screening study on early detection of lung cancer, the department collaborates with the Departments of Radiology of the UMCUtrecht and several departments in the UMCGroningen to detect early emphysema and to investigate different underlying mechanisms. Recently the department has been granted a large funding from the EU (FP 7:COPACETIC) to study the genetic susceptibility to develop COPD.
