Changing your patient information

Have you switched to a different insurance, doctor or pharmacy? Has your phone number or e-mail address changed? If so, you can change the new information yourself via My UMC Utrecht.

Change your information yourself

In the My UMC Utrecht patient portal you can change a number of details yourself, such as your contact details, GP or pharmacy. We are working to ensure that you will soon be able to change all your data in My UMC Utrecht.

Have you moved house?

If so, you can inform the UMC Utrecht of your change of address using the form below. After sending the form, we will send a confirmation to the e-mail address you entered. This confirmation contains the information you provided.

Please note:

  • We can only process changes in address details if they have also been passed on to the municipality.
  • Questions marked with * are required fields.
