Are you being admitted to hospital soon? We are happy to help you prepare properly. What should you bring with you? What questions, expectations or concerns do you have? Tell us about them so that we understand what is important to you.

Prevent spread of hospital bacteria
What will you take with you?
Always bring the following:
- Your valid identification
- Your insurance card or health insurance card
- A current list of the medications you are taking. You can pick this up at the pharmacy
- All medications you are taking in their container. Also bring medications not prescribed by your doctor, such as St. John's wort
- Do you use home health care? If so, please bring contact information for your home care
