Your patient portal on My UMC Utrecht provides direct access to data from 2015 from your medical record. This is often the easiest way to view frequently requested data. Your patient portal is accessible at any time of the day and you can save information from it. You log in to this using your DigiD. If you do not find what you are looking for there, request a copy from your medical record using our request form.

What can I find in my patient portal?

• Appointments, admissions and operations
• File (including letters, treatment reports (consultations), medications, measurements, surgery reports and questionnaires)
• Results of Laboratory, Measurements, Microbiology, Pathology (biopsy), Radiology report and nuclear tests
• Information about your Health (including allergies, implants and treatment restrictions)
• Personal information ( including contacts and consents given)

Copy of radiology images, more comprehensive information or data from before 2015 please request from file delivery team using the request form.

1. Complete the form in full;
2. Make a copy of your passport/ID card and attach it to the form*.
3. Are you requesting information about your ivf treatment that involves a partner? Then add a valid ID and signature of the partner. If you are going through this treatment using donor material, you do not need to attach anything.

Are you requesting a copy of someone else's data? Then use the request form request copy of medical record - on behalf of someone else.

*Team Uitlevering Dossier uses your personal information to locate the file and to verify the validity of your request. We also ask you to confirm your identity with a copy of your ID so that we can monitor patient privacy and security. This copy is not stored and deleted after the mandatory check.

A copy of your medical records is free once. The waiting time averages 4 - 6 weeks. The copy is sent to the applicant's (mail) address.
